Brian Holzer, MD Chief Executive Officer

Brian Holzer is a senior physician executive serving as the founder and CEO of Lacuna Health, which is a wholly owned subsidiary of Kindred Healthcare. Brian is a frequent speaker in the area of post-acute care and population health. In 2018, Brian was recognized by Health Data Management as one of the 30 top chief innovation officers at healthcare organizations as well as being selected to the “Council of 33” by the Healthcare Innovators Professional Society (HIPS).

Brian joined Kindred from Highmark Health, the third-largest integrated healthcare delivery and financing network in the nation. Most recently, Brian led the formation of HM Home and Community Services (“HMHCS”) and served as President of the organization. HMHCS has since helped drive improved clinical outcomes, better patient experience and a reduced overall cost of care for a Pennsylvania ACO and a health insurer’s Medicare Advantage line of business. Previously, Brian led the post-acute care operations at the Allegheny Health Network (“AHN”), a western Pennsylvania-based integrated healthcare delivery system under the Highmark Health umbrella. At AHN, Brian led the development and operations of AHN Healthcare@Home, resulting in a hospital system-driven continuum of care model consisting of home health, hospice, home medical equipment and home infusion.

Brian has also held positions in healthcare consulting and various strategy, operations, and sales and marketing roles in the biotechnology industry. He received a medical degree from the Drexel School of Medicine and an MBA with a major in healthcare management from the Wharton School of Business.

John Apholt Chief Operating Officer

John Apholt joined Lacuna Health in April 2018 as Chief Operating Officer. John previously served as Chief Operating Officer at Sonic Healthcare USA, the third largest clinical diagnostic company in the nation. At Sonic Healthcare USA, John had oversight for strategic partnerships and joint ventures, commercial product marketing, clinical operations, managed care contracting, human resources and payroll. John also served as President and CEO of two Division’s within Sonic Healthcare’s Federated Model, leading both of them to significant improvements in overall performance prior to becoming COO for the whole organization. Before joining Sonic Healthcare, John worked in variety of executive leadership roles in disability management, long term care, disease management, health and welfare administration and managed care. John brings extensive experience managing highly complex multi-site clinical call centers and operations to Lacuna. In each opportunity, John was able to elevate the performance of the team and achieve the triple aim of improving quality and productivity while lowering costs. John earned a Bachelor of Arts Degree from Providence College in Providence, Rhode Island.

Suzie Williams-Hayes, BSN, RN Director, Quality and Implementation

Suzie Williams-Hayes is a Registered Nurse serving as Director of Quality and Implementation at Lacuna Health. She leads the implementation of Lacuna Health products and services, as well as directs a quality team of Registered Nurses to ensure (clinical) excellence and integrity is delivered with all Lacuna Health products. Suzie, a University of Louisville School of Nursing graduate, has worked as an ICU trauma nurse, resource RN, and a transfer coordinator nurse. During her years as a critical care bedside nurse, in addition to specializing in trauma services, she worked with neurology/stroke, cardiac and burn ICU patients. Suzie also has a background in DME distribution and sales. She joined Lacuna Health in 2014, where she played an integral role in formation of a clinical outreach program (AfterCare), aimed at reducing unplanned patient returns to the acute care setting (RTAs), and is tailored to the needs of post-discharge patients. In this role, Suzie defined our Referral Coordinator Program and oversaw Placement Services. In 2017 she was promoted to a director, where she restructured our incoming call flow and clinical protocols, creating efficiencies and improving lead volumes. She also oversaw the expansion of our quality monitoring program for both the Call Center and Placement Services. In addition, she has also led the implementation of multiple call center products focused on the needs of hospital, home health and hospice patients.

Fata Dzeko Director of Operations

Fata Dzeko is an experienced professional serving as Director of Operations at Lacuna Health. Fata is responsible for overseeing the day-to-day operations of a rapidly growing team of Nurse Advocates and Care Coordinators who provide inbound and outbound telephonic services. Fata has fourteen years of call center experience with a focus in healthcare. She started her career at the University of Pittsburgh Medical Center (UPMC). While at UPMC, she was responsible for day-to-day operations of a 24/7 after-hours call center servicing the UPMC Physician Services Organization. There, Fata oversaw significant improvements in average handle time, workflow efficiencies and customer issues. In addition, she successfully led many telephonic outreach campaigns. Fata was a 2010 Fine Awards for Teamwork Excellence in Health Care bronze medal recipient for her role in the UPMC’s Patient Quality Outreach Program awarded by the Jewish Healthcare Organization.

After 10 years at UPMC, Fata joined Allegheny Health Network (AHN), a western Pennsylvania-based integrated healthcare delivery system. While at AHN, Fata oversaw the formation and operations of AHN Centralized Physician Scheduling Call Center and played a key role in converting the Physician Services Organization to the Epic electronic health record (EHR). Prior to joining Lacuna Health, Fata served as a Director of Operations for AHN Healthcare@Home Home Health and Hospice central intake and scheduling departments where she optimized scheduling capabilities and developed and implemented the new hire onboarding and quality assurance programs.

Fata is a graduate of Robert Morris University, where she earned her BS in Applied Mathematics. She is currently working on earning her MBA from California University of Pennsylvania.

Marvin McLellan Head Business Development

Marvin McLellan joins Lacuna Health with three decades of experience in business development, strategic planning, and healthcare operations leadership. McLellan most recently served as vice president, business development at Medical Life Holdings, a portfolio of value-based healthcare companies where he had oversight of all business development, strategic partnership and growth activities for three companies. Marvin also served as market director of business development at Tenet Healthcare, where he was responsible for business development, physician relations, marketing, strategic planning and growth initiatives in the El Paso, Texas region.

Prior to joining Tenet, McLellan also served in senior leadership roles at Air Methods Corporation where he was the architect of Direct Patient Logistics, a national patient transfer call center presently serving over 200 hospitals and over 300,000 patient transfers a year.  He also led MediFlight Oklahoma, the critical care transport service for OU Medical Center and served in senior operations leadership roles at TransCare New York and Northwell Health, formerly North Shore LIJ Health System.

Marvin has a diverse educational experience that includes critical care paramedic licensure, a bachelors in business administration and is currently completing a curriculum to achieve an MBA in international healthcare administration at the University of Cambria in Carlisle, UK.  

Amy O'Neill Director, Finance and Data Analytics

Amy O'Neill serves as the Senior Financial Officer for Lacuna Health with oversight of the operational finance, analytics and reporting teams.    Amy brings a wealth of knowledge to Lacuna Health, with over 25 years of experience in progressive financial leadership roles at blue chip companies.  She spent 17 years at Humana Inc., a Fortune 100 healthcare company, where she built and managed one of the top analytic teams in the company.  Amy participated in Humana’s Women in Leadership and Succession Management Programs. 

Throughout her career, Amy has worked extensively on budgeting, forecasting, analytics, new process development and execution and team building.  She is also an expert in the development of strategic, operational and financial reporting and metrics.

Amy graduated from the University of Kentucky, where she earned a BA in Finance.  She resides in Goshen, KY with husband, Dennis, and their two children.  She enjoys reading, cooking, traveling, spending time with her family and cheering for her beloved Kentucky Wildcats.
